§ 406-11. Application for Certificate of Authority.

  • Application for certificate of authority or permit, under this chapter, by any motor transportation company, shall be made in writing to the director, setting forth upon oath the following:

    (a)

    A statement showing the principal office or place of business of the applicant.

    (b)

    A statement giving full information concerning the physical property used or to be used by the applicant.

    (c)

    The make and name of the manufacturer of the motor buses to be used.

    (d)

    The state vehicle license numbers of such motor buses.

    (e)

    The factory number of such motor buses and the year of their manufacture.

    (f)

    The name of the owner or owners of such motor buses and a copy of any mortgage, conditional sale contract, or agreement, relating to such motor buses.

    (g)

    The seating capacity of such motor buses.

    (h)

    A complete statement of the proposed routes over which the applicant desires to operate.

    (i)

    The proposed time schedule or schedules or timecards of the applicant.

    (j)

    The proposed tariff showing the passenger rates to be charged between the several points.

    (k)

    A suitable sketch map showing the route over which the applicant desires to operate.

    (l)

    Copy of partnership agreement, if more than one person interested and not incorporated, or, if a corporation, the names of the officers of said corporation.

    (m)

    The capital stock of applicant, if it is a corporation.
