§ 00077-3. Site Review Application, Installation Permit, Alterations Permit, Operation Permit, Abandonment
A.
No person intending to install a new Household Sewage Treatment System or replace an existing Household Sewage Treatment System shall be issued an installation permit without the Cincinnati Board of Health first approving a site review application.
B.
A site review application for a Household Sewage Treatment System alteration may be required by the Board of Health, and when required, shall contain all pertinent information and the application fee required by the Board of Health. In the case where an alteration involves the expansion of a soil absorption component, the Board of Health shall determine when a site and soil evaluation shall be conducted or required in compliance with section 3701-29-08 of the Ohio Administrative Code.
C.
No person shall install or replace a Household Sewage Treatment System without an approved and valid installation permit issued by the Cincinnati Board of Health. No person shall alter a Household Sewage Treatment System without an approved and valid alteration permit issued by the Board of Health.
D.
The owner or his/her designated agent shall obtain an installation permit from the Cincinnati Board of Health for the installation of a "Household Sewage Treatment System" prior to the start of construction of a dwelling.
E.
An approved installation permit or alteration permit issued by the Board of Health shall be valid for one year from the date of issuance or until the installation or alteration is completed and approved by the Board of Health within the one year period. The Board of Health may extend the permit period for an additional six months for permits issued pursuant to the Ohio Administrative Code.
F.
No person shall maintain or operate a "Household Sewage Treatment *9* System" without an operation permit from the Cincinnati Board of Health.
G.
Every operation permit shall expire on December 31 of each year, irrespective of the date of issuance, and shall not be transferable. All operation permits shall be renewed before January 1 of each year.
H.
A permit to abandon may be required by the Board of Health for any person who is no longer using a Household Sewage Treatment System or an applicable component of a Household Sewage Treatment System to properly abandon all tanks, dosing tanks, and/or pretreatment components that are no longer in use.
